Resident Services Assistant
Shifts Available: varied shifts
The Gary Residence is seeking a Resident Services Assistant to provide care to our residents. This position entails providing support to residents with activities of daily living including personal hygiene, socialization, cognition and physical health. The ideal candidate for this position will exemplify The Best Friends™ approach to dementia care by utilizing the essential elements of friendship (respect, empathy, support, trust, and humor) to care for residents and adapt to their remaining strengths and abilities.
Responsibilities:
- Assist residents with activities of daily living, including: toileting, bathing, dressing, grooming, mobility, or transport.
- Taking and documenting vital signs.
- Light housekeeping, including: making resident beds, removing trash from resident apartments, folding resident laundry, keeping common areas clean and tidy, etc.
- Encourage residents to attend and participate in a variety of activities.
- Observes for and informs Medication & Resident Services Assistant promptly about any changes in resident health/condition, behavior or needs.
- Practices patience, compassion, and empathy in all interactions with residents.
- Focuses on residents’ abilities and provides social/emotional support and encourages the resident to be as independent as possible.
Qualifications:
- Experience interacting with elderly with memory care needs.
- Willingness to learn and complete on-the-job training and utilize The Best Friends™ approach to dementia care.
- A desire and commitment to teamwork and effective communication.

Medication & Resident Services Assistant
Shifts Available: all shifts
The Gary Residence is seeking a Medication & Resident Services Assistant to provide care to our residents. This would entail administering routine medications and treatments to residents in accordance with facility policy and procedures, federal and state regulations. The ideal candidate for this position will exemplify The Best Friends™ approach to dementia care by utilizing the essential elements of friendship (respect, empathy, support, trust, and humor) to care for residents and adapt to their remaining strengths and abilities.
Responsibilities:
- Assist residents with activities of daily living, including: toileting, bathing, dressing, eating, toileting, hygiene, and mobility.
- Administers medications and observes for additional care needs or changes in residents’ usual behavior, condition, or functional abilities.
- Reports observations to the nurse in a timely manner.
- Regularly performs a medication cart audit.
- Document information pertaining to changes in residents’ functional status, personal needs, appointments, etc.
- Practices patience, compassion, and empathy in all interactions with residents.
- Focuses on residents’ abilities and provides social/emotional support and encourages the resident to be as independent as possible.
- Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention under the instruction of the nurse.
- Assist in care of Hospice Program.
Qualifications:
- Experience interacting with elderly with memory care needs.
- Willingness to learn and complete on-the-job training and utilize The Best Friends™ approach to dementia care.
- A strong sense of compassion and a desire and commitment to teamwork and effective communication.
- Experience with med-passes.
- Preference given to LNA’s with experience, however, we are willing to train the right candidate.
- Must pass appropriate background checks and be fully vaccinated against COVID-19.

Resident Care Director Assistant (LPN)
Job Type: 32 hours a week, position in Clara’s Garden Memory Care Neighborhood
The Gary Residence in Montpelier, VT is seeking a Resident Care Director Assistant (LPN) to provide care to residents with memory care needs. The LPN is responsible for supporting the RN. The preferred candidate will be eager to support our mission to provide a high-quality residential setting for senior individuals which promotes independence, individuality, choice and security while supporting and enhancing the quality of life for the population it serves.
Responsibilities:
- Support the RN The Gary Residence.
- Provide medical input for residents, families, and staff.
- Assist in the development and update of each resident’s care plan by assessing residents’ functional status on an ongoing basis.
- Document information pertaining to changes in resident’ functional status, personal needs, appointments, etc.
- Assure that assistance plans are completed and arrange services and support to meet resident needs.
- Keep Executive Director aware of problems, concerns and updates of each resident and enlist her assistance as needed.
- Review all emergency calls for assistance by residents and complete a quality-action review on the treatment and handling of each emergency by staff.
- Carry out other duties as assigned by supervisor or as necessary to maintain quality and continuity of services and safety of residents.
Qualifications:
- Active LPN Vermont State license.
- Two to five years’ experience providing resident care to the elderly, or similarly related experience with specialized memory care programs.
- One year experience with resident assessment and care planning.
- Understand and conceptualize complex concepts, as well as ability to make appropriate decisions in stressful situations.
- High level communication skills, both verbal and written.
- Ability to show sincere compassion towards cognitively impaired older adults.
- Demonstrates genuine concern for the physical and emotional needs of the elderly and their families.
- Must be proficient in basic office equipment, including a personal computer, for the purpose of accomplishing and maintaining a high level of job performance.
- Must pass appropriate background checks and be fully vaccinated against COVID-19.
Pay: $60,000.00 – $65,000.00 per year
Residential Care Director (RN)
Job Type: Full-Time
The Gary Residence is seeking a team-oriented RN with experience providing care in a residential care setting. The ideal candidate will have interest and knowledge providing care to residents with memory care needs. The RN is responsible for coordinating resident care to ensure the residents remain healthy and active for as long as possible. The nurse is a liaison with hospital personnel, physicians, community organizations, and other health related service agencies to provide care to the residents. The RN is responsible for practicing according to the State Board of Nursing within the scope of practice for his or her license and always in compliance with The State of Vermont.
Responsibilities:
- Conducts pre-admission evaluations of all prospective residents.
- Ensures provision of services to the residents and ensures that all state policies governing resident care are closely monitored and carried out.
- Evaluate, motivate, and monitor the performance of the care staff in the community.
- Ensure the completion of assessments on all residents once per year, as required by State of Vermont licensing requirements.
- Oversees continuing education programs for direct care staff in the community, as regulated by State licensing requirements.
- Conducts orientation for new team members of the department to ensure that all employees are knowledgeable of the goals, policies, and procedures of the department.
- Conduct interviews/observations on a continual basis of residents to alert Executive Director, family, and other appropriate individuals of any changes in the physical or emotional health of the resident.
- Maintains documentation in resident files.
- Act as care manager and resource person for the residents and their families.
- Measures outcomes, set goals and leads the care team to achieve those outcome goals
- Assume on-call responsibilities on a rotational basis and assign on-call responsibilities to other employees on a rotational basis to ensure after-hours response to resident emergencies.
- Carry out other duties as assigned by supervisor or as necessary to maintain quality and continuity of services and safety of residents.
Qualifications:
- Active RN Vermont State license.
